Direct Primary Care (DPC) Membership

Costs are rising everywhere and Revolution Health desires to deliver high-quality health care with a Functional/Integrative influence in as cost-effect manner as possible. Unfortunately, increasing prices have affected us as well and we want to do what we can to save each patient as much money as possible yet allow the clinic to remain financially viable.

In order to achieve these goals, Revolution is proud to introduce our Direct Primary Care (DPC) Membership. This Membership, in exchange for an annual fee (there is an option to pay monthly), allows us to ensure a higher-quality while providing cost savings to you, our patient.

Pricing

A Monthly Fee will be billed each month based on your age. Spouses & Kids receive a special discount.

7-17 years old (if a parent is a Member)

We are happy to provide Primary Care to patients ranging from 7 to 17 years of age. The Annual Fee is $300 per kid when they have a member parent.

18-22 years old (if full-time student or living with parent)

Dependent children of DPC Members can also be members. The Annual Fee is $780 if they are either a full-time student or if they are living with their parents.

18-39 years old

Patients in this age range receive an Annual Preventive Physical Exam consisting of the items listed below. The Annual Fee is $1380. Spouses can join at a reduced rate of $1020 per year.

  • Spirometry
  • Heart Rate Variability (HRV) Test
  • Vision Screen
  • Depression & Anxiety Screening
  • Recommended Bloodwork*

40-65 years old

As we age we generally need more medical evaluation and intervention to maintain optimal health and prevent illness & disease. We recommend additional screening starting at age 40. These patients will also receive an Annual Preventive Physical Evaluation with some added components. The Annual Fee is $1620. Spouses may join at $1200 per year.

65 years old and over

Patients 65 years and older generally require the highest levels of testing, screening, and management which further increases costs. Therefore, there is a slightly higher Annual Fee at $1860. Spouses may join at $1440. The Annual Preventive Physical Evaluation is as listed above for patients 40-65 years of age.

* – A limited formulary of medications is available at Revolution Health & Wellness Clinic at a reduced price. We have no control of any medications not provided at Revolution Health directly.

** – Bloodwork labs are not included in the Membership Fee. Pricing depends on labs ordered, your insurance, deductibles, co-pays, etc.
