Chad P. Edwards, DO – Medical Director, Founder
He enlisted in the US Army in 1989 as a medic and preventive medicine specialist. He was a Drill Sergeant at Fort Sill, OK for three years.
He holds a B.S. in Exercise Science & Sport Medicine from Oklahoma Baptist University and attended Medical School at Oklahoma State University College for Health Sciences. He completed residency at Womack Army Medical Center in Fort Bragg, N.C. and is board certified in Family Medicine.
He served as the Battalion Surgeon for a Special Operations unit in support of Operation Iraqi Freedom (OIF). He was a Flight Surgeon and a Dive Medical Officer in the US Army and had the distinct honor of working with and serving in several US Army Special Operations units.
Dr Edwards has worked as an Emergency Physician in several hospitals in Tennessee and Kentucky. He is board certified in Family Medicine by the American Board of Family Medicine. Upon his departure from the Army, he moved back to Tulsa and worked at Warren Clinic for 1 year.
Dr Edwards is the Medical Director for the Patriot Clinics which specialize in the treatment of Traumatic Brain Injury (TBI) and Post-Traumatic Stress Disorder (PTSD) using HyperBaric Oxygen Therapy (HBOT). He is also the Medical Director of the Full Circle Health Clinic in Edmond, OK.
Recognizing the need for a truly functional medicine clinic in Tulsa he started Revolution Health & Wellness Clinic. Here, he pays special attention to sports medicine and health & wellness as a preventive strategy to optimize health, minimize injury risk, and decrease disease.
Viewing his patients through a holistic approach helps identify subtleties in their function that are beginning to show signs of dysregulation. His favorite thing in medicine is helping others achieve their dreams of feeling healthy again when nothing else has been able to help.
He enjoys running and working out. He is a Level 1 CrossFit Trainer.
Chrystal Lewis, DC
Dr. Chrystal Lewis is an Upper Cervical Chiropractor that is passionate about helping others to restore their lives through spinal alignment, functional lifestyle management and education. At a very young age, Dr. Lewis knew that she wanted to be a doctor. She received her Bachelors of Science degree from the University of Tulsa and her Doctorate of Chiropractic degree from Logan College of Chiropractic in St. Louis, MO. Dr. Lewis brings with her more than decade of experience. After graduating from chiropractic school, she was an associate at another upper cervical practice for 2 ½ years before opening her own practice in 2006. Dr. Lewis has also teaches multiple subjects at the local community college where she enjoys teaching students about health and wellness. She is now excited to join the Revolution Health team and looks forward to helping more people on their journey to optimal health.
Dr. Lewis practices an upper cervical technique, called NUCCA (National Upper Cervical Chiropractic Association). This technique is a very gentle, non-invasive technique that does not involve any popping, cracking or twisting. NUCCA is gentle enough to use on everyone, even newborns and the elderly. The technique is called upper cervical because it utilizes the top bone in the spine (C1/atlas) as a lever to align the spine, as well as the pelvis, hips, shoulders and skull. This restores balance to the entire body and neurological system and allows for maximum potential in healing. Since the body is controlled by the nervous system, keeping the spine in proper alignment allows the nerves to function at the highest level, removing interference from the communication between the brain and the rest of the body.
Dr. Lewis is a native of Tulsa, OK. She is married and has a beautiful daughter that keeps her delightfully busy. She volunteers at her local church and loves spending time with her family, hiking, running and lounging on the beach, when possible.
Nicole Forsberg, APRN-C, RNFA
Nicole is a nationally certified Family Nurse Practitioner with whose credentials include academic achievement and extensive and diverse practical experience.
Nicole has been a registered nurse since 1996, when she graduated from Oklahoma Wesleyan University. She began her career working for a general surgery practice in Carson City Nevada, in addition to gaining experience in Northern Nevada hospitals.
During her matriculation at the University of Nevada Reno Nurse Practitioner program she worked with two preeminent Reno, Nevada plastic and reconstructive surgeons. Nicole is also a Registered Nurse First Assist and has assisted in both elective and trauma surgical procedures in which she gained extensive knowledge and experience in diagnosing and treating conditions of the skin and in performing an array of cosmetic procedures.
After her gradation with honors from the UNR Family Nurse Practitioner program, she eventually accepted a position with Sierra Regional Spine Institute. Nicole was mentored for years by a respected orthopedic spine surgeon and physician to the U.S. Ski Team.
Nicole is a native of Jenks, Oklahoma, and always longed to return home to be close to her large and beloved family. In searching for the right professional opportunity in the Tulsa area, she accepted and worked in a successful and thriving dermatology practice. Nicole has always wanted to broaden the scope of her practice and found the perfect fit with Dr. Chad Edwards in his Functional Medicine practice at Revolution Health & Wellness Clinic. Nicole shares Dr. Edwards’ commitment to providing his patients with high quality patient care and personal attention they deserve.
Nicole is married and the mother of two beautiful, talented boys, Gabe, age 16 and Luke 15.
Linsey Lees, BSBA, BSN, RN, Certified Nurse Injector
Linsey Lees grew up in Collinsville, Oklahoma. Linsey obtained her bachelors degree in business management from Rogers State University while playing collegiate softball for four years. From there Linsey attended The University of Oklahoma in Tulsa, Oklahoma to obtain her bachelors degree in nursing.
Linsey’s passion is serving her patients with quality of care paying attention to detail. Linsey loves making people feel better about themselves through being a certified nurse injector. Linsey seeks to educate clients on the benefits of receiving botox and filler injections.
Linsey says, “The thing I see most often with my clients is them being so nervous to try something for the first time. I want everyone to feel comfortable with treatments. Many clients tell me they wish they had started treatments sooner, truly loving their results. I enjoy enhancing others natural God given beauty.”
Linsey has joined Revolution Health and Wellness team collaborating with Dr. Chad Edwards adding the benefits of aesthetic treatments all in the same location.
